‘My Platform’ will give you access to a diverse range of training resources to ensure you have the right skills and knowledge related to your role, continuing to deliver the best possible service to our local population. The course consists of 5 lessons to learn about how to use Fr E D A in SystmOne, as an effective efficient tool to help proactively identify all people with frailty, dementia and any aged adult deemed likely to be within last year of life (end of life), deliver holistic assessments based on Comprehensive Geriatric Assessment (CGA) framework, and serve as an effective Personalised Care and Support Planning (PCSP) tool.
